Shore Medical Center Community Health Needs Assessment

Shore Medical Center is dedicated to improving the health of people who live, work, and vacation in our community. Throughout its history, Shore has recognized its role in providing programs and services that focus on access to health care, prevention, and health improvement.

Every three years Shore Medical Center is required to perform a community health needs assessment to identify gaps and needs of the community so we can achieve health equity among all populations. We are currently in the survey process.

Click here to complete the 2022 Shore Medical Center Community Health Needs Assessment Survey. This brief survey is designed to be completed in 2-3 minutes. Our goal is to have all surveys completed by July 31, 2022.

Thank you for taking a few moments of your time to help us identify the health needs of our community. From these surveys, we will develop an implementation strategy to identify Shore Medical Center’s plan of action for community benefit projects for 2023-2025.

A Bit About the Coalition

Cape Atlantic Coalition for Health is a regional chronic disease prevention coalition funded by the New Jersey Department of Health, Office of Cancer Control and Prevention. Our purpose is to reduce the state's cancer burden and improve health outcomes for people with, or at risk for, cancer and other chronic diseases in Atlantic and Cape may counties. For more information on comprehensive cancer control, visit www.njcancer.gov. Shore Medical Center is the lead agency for Atlantic and Cape May counties.
